A herpes lesion swab: why healing and timing matter

A fresh genital blister or ulcer may offer a useful opportunity for a clinician to collect a herpes lesion sample. Healing changes what a swab can detect, particularly for viral culture. Do not delay assessment just to see whether an online photograph begins to resemble your lesion more closely.

Describe the active lesion and its timeline

Explain when the lesion appeared, whether it began as a blister and whether there is pain, urinary difficulty or a previous similar episode. Mention pregnancy, immune suppression and any medicine already used. A clinician can decide whether a lesion NAAT, often called PCR, or culture is appropriate and whether other causes of ulcers need investigation. Appearance alone cannot safely distinguish every infection or non-infectious skin problem.

Understand what healing changes

Culture depends on recovering viable virus and becomes less sensitive as a lesion heals. Molecular lesion tests are generally more sensitive, but no negative result can be interpreted without the lesion stage and collection circumstances. A negative sample from an old lesion does not prove that herpes has never been present. Do not open, scrape or deliberately irritate a healed area at home to create a new sampling surface.

Plan what happens if no lesion remains

Ask how to obtain assessment if another episode occurs and whether any different diagnostic approach is justified in your circumstances. Random genital swabs without an active lesion can miss infection; they are not a reliable replacement for targeted lesion testing. Blood antibody tests answer a different question and have their own limitations. Severe pain, inability to pass urine, eye symptoms or significant illness warrants prompt care rather than waiting for a more convenient swab appointment.

Common questions

Is a healed-lesion negative result definitive?

No. Healing, method and collection affect what a negative result can exclude.

Should I break a blister myself before attending?

No. Leave assessment and specimen collection to the appropriately instructed clinical team.

Is a random swab without lesions equivalent?

No. It can miss intermittently shed virus and does not replace a targeted diagnostic plan.
